## Narrative

A roof fall occurred at 37 block intersection 23 West track, approximately 27' long, 30' wide and 9' high. The fall was caused by a geologic abnormality of a layer of unforeseeable fractured shale above the anchorage point of the resin assisted point anchor bolts. The fall will be cleaned up & supported. No problems with mining machines, methods, PPE, training or compliance issues.
