Narrative

The employee did not have an accident or incident. He was diagnosed with bilateral carpal tunnel syndrome on October 10, 2014 utilizing a Neuromuscular and Electro diagnostic Medicine Consultation. Employer was informed on Monday, December 15, 2014 that employee had underwent right carpal tunnel surgery on November 11, 2014 and left carpal tunnel surgery on December 15, 2014.
