Narrative

The employee stated that while hanging high voltage cable, a chain he was using became fouled, when freed, it swung around and struck him in the mouth, resulting in a chipped top front tooth. The injury was caused by the employee's inattention to the use of the chain instead of a softer material like a nylon rope.
