Narrative

Employee was putting drill steel in drill chuck when the drill operator closed the clamp on the helpers hand causing a fracture to his left middle finger. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.
