Narrative

employee had been experiencing pain in his lower back for approximately a year. MRI report revealed disc dessication and degeneration and two disc extrusions related to cumulative trauma from lifting and handling heavy longwall equipment. Employee had a physical therapy appointment on 12/29/14.
