## Narrative

Employee was disconnecting a supply hose from rock dust pod that had been left hooked up to dust pod from previous shift in which time allowed neck of dust pod to be packed with rock dust.  The clogged neck prevented the supply hose from being relieved of pressure and when he started removing the supply hose it blew off striking him in the face causing a fracture and lacerations.
