## Narrative

EE states numbness and mild burning sensation in both wrists.  He was evaluated by a Physician and was given a diagnosis of Carpal Tunnel Syndrome with surgical repair being done on 10-31-14.  It was made known that work activities were a contributing factor on 1-2-15.  The employee retired on 12-31-14.
