## Narrative

Employees were removing the drive sheave from the agitator on the pack cell. They removed the bolts that hold the tapper bushing to the sheave. One employee installed the jack bolts that press the bushing & sheave apart. As he was tightening the bolts the pieces separated & both dropped to the bottom guard plate trapping the employees finger between the sheave & the plate.
