Narrative

The employee claims that while walking along the belt, he stepped in hole in the pavement, tripped and fell, injuring his left foot. The accident was caused by the failure of the employee to observe, recognize the bottom conditions and take the appropriate preventative measures. The employee was in compliance with rules, regulation and policies.
