## Narrative

EE strained his back while working at Lime Load-out station.  He aggravated it further while adding oil to plant equipment. EE was seen by a Chiropractor and placed on leave for 3 days for a stained back.  The operator did not receive physician's exam write up and RTW clearance until 01/08/2015.
