Narrative

The employee was using a 3/4 inch drive to remove a bolt from a gear box. The bolt sheared off when the miner was applying torque and the miner lost his balance and fell, striking his shoulder. The following were not a factor: Rules, Regs., protective devises or clothing, training, proficiency, skills or attitude.
