Narrative

Ee saw doctor on 11/19/14 for ongoing pain in both hands. He was referred for further testing to determine if there was work related occupational illness. On 01/26/15, it was confirmed that the ee had bilateral cubital tunnel and left wrist carpal tunnel syndrome that was work related. Surgery scheduled on 02/06.
