Narrative

Employee was lowering jack, in the process of re-railing a mantrip. Employee failed to keep firm grip on jack bar as instructed. Resulting in jack bar coming out of jack striking employee behind right ear resulting in laceration. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.
