## Narrative

AMS was contracted to install new water supply piping.  In the process of moving a section of pipe with a forklift one of the forks became stuck underneath a small metal lip.  The forklift operator attempted to tilt the forks to load the pipe unaware that a fork was caught.  When the fork released it caused the pipe to elevate off the forks and strike the worker in the chin.
