Narrative

The employee stated that while attempting to align a pin in the door of the side roof bolter of a continuous miner, with his finger, the door moved lacerating his fingertip. The accident occurred because the employee used his finger to check the door alignment. This is not the way the employee was trained to check the pin alignment.
