Narrative

Employee was installing bolt next to rib. Vibration of drill caused rock to separate and fall striking left lower arm resulting in a small laceration. Cause of the injury was body placement. PPE was not a factor, compliance rules/regs was a factor, mining equipment/systems was a factor, miner proficiency may have been a factor.
