## Narrative

While entering the long wall face from over the tailgate sail backwards. He turned around, a piece of screen hanging down between shields caught employees glasses and scratched left eye. Cause of accident was screen between two shields. PPE may have been a factor, compliance rules/regs was a factor, mining equipment/systems was not a factor, miner awarness was a factor.
