Narrative

Individual was removing a broken piece of mono-rail when the dolly rolled pinching the right index finger, due to lack of concentration resulting with a laceration and fracture to the finger tip. This did not involve any compliance issues, no equipment involved. This did not involve proficiency or attitude. All personnel safety items were being utilized.
