Narrative

While doing rehab work employee was running miner when a rock was thrown 5' past miner striking victim in the left foot. Cause of the accident was 8' bolt caught bit lug and threw rock rearward. PPE was not a factor, mining equipment/systems was a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.
