## Narrative

The east skip of the #3 production hoist was positioned at the collar and un-clutched. The west skip was then positioned at the collar and brakes set. The electrical power was disconnected. The west skip began to slip in the brakes and moved down the shaft, going to the bottom. The hoist rope broke and both the hoist rope and skip landed at the shaft bottom.
