Narrative

EE was pre-op prepping Hyster forklift. He slipped on ice, landing on left side. 3.10.15 diagnosed w/rotator cuff inj. by Dr. taken off work. Previous left rotator cuff repair on 3.28.01 MRI revealed moderate atrophy of supraspinatus & infraspinatus tendons & moderate atrophy of respected muscles. we believe prior Sx contributed to most recent tear & submit as occupational illness.
