Narrative

Employee was pulling belt on HG 3 storage unit. Had belt bar through belt and chained on both sides to 1/2 wire rope. The belt got hung up causing the bar to bend. Upon bending the chain come off one side causing the bar to spin around and strike the employee on arm.
