Narrative

Mechanic was lifting a tire jack onto the flat bed of a truck. He felt a pain in his left shoulder and returned to work. He did not report the injury until the following day. He was seen by a doctor on 7-31-14 and was given a full duty release. On 9-5-14 he was given a light duty release in response to an MRI indicating a torn rotator cuff.
