Narrative

Individual was using Chain Tuggits to lift a 636 scoop canopy when the canopy twisted the Tuggit chain hook came dislodged from the chain allowing the canopy to fall striking the right hand. (laceration to the palm of little finger) This did not involve any compliance issues, job skills, training or attitude. Personal safety equipment was being used.
