Narrative

Employee was using pry bar to straighten cable handler. The pry bar slipped causing employee to fall on right ankle causing fracture.(Lost Time as of 04/03/2015). Compliance with rules and regulations was not a factor. Miners proficency and job skills may have been a factor.
