Narrative

Employee was operating 55 jeep and struck 43 locomotive outside on the old jump track. The accident occurred because the employee wasn't paying attention. A state inspector checked the jeep after and found no compliance issues that would of resulted in accident It didn't involve job skills proficiency training or attitude. No PPE or protective devices on equipment involved.
