Narrative

Employee was in the process of unloading a CLA pot for a Joy miner when it shifted and fell on his left foot resulting in three fractures. The accident occurred due to improper body placement by the employee. It didn't involve any compliance issues. It didn't involve job skills,proficiency,training or attitude. No PPE or protective devices on equipment involved.
