Employee has felt worsening pain in his back. Surgeon diagnosed the employee with two-level degenerative disc disease @L4-L5 and L5-S1. Employee had surgery on 2/16/15 although the pre and post surgical notes were only received by the operator on 4/6/15 and determined that the injury was likely work related.
