Narrative

The miner was removing a 2 inch depressurized hose. When the miner pulled on the hose and it came free and stuck him in his lower lip causing a laceration. The miner received 5 stitches. the following were not a factor. rules, regulations, mining systems, job skills, proficiency, training, attitude, clothing or protective devices.
