Narrative

Employee was standing on a ladder hooking come-a-longs in monorail when a part of the rib fell out pinning his leg against the ladder causing injury to his knee and shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOSING TIME UNTIL 4/8/15 DUE TO INCREASED PAIN IN SHOULDER.
