Narrative

EE was at Cumberland Portal attending safety training. During class break he was walking to the rest room located next to the training room. EE was conscious, alert, first aid administered, transported to ER. Small laceration required 2 staples. Attached medical information show dizziness caused by syncope. No act, no condition please consider OI.
