Employee entered entry 1 to follow up on cleaning. 12 - 15' inby the xcut he heard the roof pop, looking up but not seeing anything he went on. A rock 73""x48""x4"" fell from between the center bolts, knocked him to the floor and landed on his left foot, fracturing 4 toes and causing the amputation of his pinky toe.
