Narrative

EE was helping install a rib bolt. While pushing bolt, EE pinched Rt index finger between a 6'x 6' plate and a ""pie pan"" causing a laceration. It was determined that EE needed surgery on injured finger. EE had surgery on 4/29/15. EE is now off work for recovery. This is being submitted as a lost time accident.
