Medical evidence received on 05/04/2015 that is dated 05/01/2015 indicate that this retired former employee had received an award for Occupational Pneumoconiosis. His last day of exposure was 08/20/1998.  This incident was caused by the employee's failure to wear respiratory protection.  The employee did not violate any rules, regulations or policies.
