Medical evidence received on 5/13/2015 that is dated 5/11/2015 indicate that this retired former employee had received an award for Occupational Pneumoconiosis. His last day of exposure was 11-28-2014. This incident was caused by the employee's failure to wear respiratory protection. He did not violate any rules, regulations or policies.
