Narrative

During the reassembly of #1 Sec. Crusher hopper employees were on 4th floor guiding hopper into place. EE was checking east end of hopper; bolt holes were not lined up. As it was being raised EE placed hand on live frame scalping screen. Hopper came in contact with middling chute causing pinch point. EE sustained lacerations
