Narrative

EE was walking up #1 entry on #6 unit to work on a mine curtain for the continuous miner. As he was walking on uneven ground, he fell down on his right elbow. He continued to work but complained of constant pain in the arm. After being sent to see an orthopedic surgeon, he has been taken off work and has been scheduled to have surgery on 5/22/15.
