The employee reported to his on supervisor on 3/4/15 that he was experiencing pain in his elbow while taping a cable. The employee has 4 years of repetitive motion of the elbow performing maintenance activities such as pulling/pushing wrenches, swinging hammers, pull on cables/wires etc. He started missing work on 5/15/15.
