## Narrative

Approx.6:00 pm 12/30/14 employee was disassembling a vulcanizer to set it up on a conveyor belt where a repair had been made. Splice needed to be heated to complete process.  Employee removed his gloved for approx.5 min. to loosen bolts. Weather conditions were extremely cold (sub zero). On 1/1/15 employee went to E/R in UT due to pain in hands/fingers. Diagnosis - Frostbite.
