## Narrative

(IE) was roof bolting on Unit 2. He had just pushed an 8ft. roof bolt up into its hole when he felt pain and a pop in his left forearm. He was transported out of the mine for medical attention. He was examined at 4 different medical facilities before having surgery on May 28, 2015 to repair a partial thickness tear to his left distal bicep.
