## Narrative

Customer bulk truck EE was preparing to climb on tank trailer to open the lids for loading.  He was at the tie-off rack at Carmeuse and had hooked to the self retracting lanyard.    As soon as he placed his second foot and full weight onto the step, (This step is 20-1/2 inches from the ground.) it broke and he fell to the ground, landing on his buttocks.
