A fully trained miner was hooking up hose to drop 226 shield the 1/2"" plug had a staple that was difficult to remove the employee struck the staple in the plug with a hammer the plug popped out while there was stored pressure in the line striking the employee on the head with the plug and getting emulsion in his left eye
