Narrative

A fully trained miner was rockdusting 14 west belt when the hose became clogged the employee tied off the rockdust hose and starting shaking the hose to unclog the hose the pressure built up because the valves were set wrong on the rock duster and the coupler came apart and the hose struck the employee in the left shoulder and sprayed rockdust in the employees face and eyes.
