## Narrative

The employee stated that while guiding a roof bolt through a roof strap with his right hand, the mining machine moved forward pinching the fingers of his right hand between the roof bolt and the roof jack.  The accident occurred because the employee failed to keep his hand out of a pinch point area and off the moving roof bolt.  He had on his required PPE.
