## Narrative

Individual was dragging a 1"" wash down hose to clean the 1st. floor of the  prep-plant floor. Due to lack of concentration EE slipped and experienced pain in the right groin and right lower back areas. This did not involve compliance issues, no equipment involved. All personal safety items were being utilized.
