## Narrative

After delivering concrete to mine, employee's foot slipped on the combined sand & concrete build-up that was around the pipe that the employee was pouring concrete into, causing him to fall.  When falling he stretched out his right arm & hand to catch himself & his right shoulder became dislocated when his hand hit the ground.
