Narrative

EE was talking to another employee when she stepped back into a clean up chute opening. The EE's left leg fell all the way through the chute; a bar to guard the chute opening was missing. The ee landed on their bottom with their right leg out of the chute. EE reported tightness and discomfort in lower back. Physical therapy was prescribed on 06/24/2015.
