Narrative

While prying out a staple from a hydraulic hose connection, the bar slipped pinching the tip of the left ring finger against a stack of wedges. The accident occurred because the employee failed to be observant of his hand placement. The accident did not involve any aspect of compliance with rules or regulations, nor mine equipment or the mining system; nor job proficiency.
