## Narrative

Employee was using a pair of chain hoists to pull in belt on 4 South. In the process of operating the chain hoist the clamp came loose off of the belt and caused the employee to fall off the ""A"" frame where the employee was standing beside of the rock box. This caused the employee to strain his right knee, ankle, and put a small bruise on his face.
