Narrative

Employee reported to us on 7/14/15 that his R. shoulder had some ongoing aches & discomfort associated with it. He thinks on/or about 7/6/15. There was no event, no injury, no trauma to it. After going on days off, then reporting back to work on 7/14/15~ he THEN informed us of this. We took him to medic, then to doctor. Restrictions were issued, in which we can work with.
